The same kidney function shows up as a higher creatinine in a muscular person and a lower one in a frail or low-muscle person, because creatinine comes from muscle. That is why it is fed into the eGFR equation with your age and sex. It can also sit inside the reference range until roughly half your filtering capacity is gone, the gap clinicians call the creatinine-blind range, so one clean reading is a start, not proof your kidneys are fine.
Creatinine is a waste product your muscles shed around the clock, and healthy kidneys clear it from your blood, so the level rises as filtering falls. The catch is that a bigger, more muscular body makes more creatinine to begin with, so the same kidney function reads higher in a heavily muscled person and lower in a frail or low-muscle one. That is why creatinine is never scored on its own. It is the raw input to eGFR, the figure your lab actually scores, computed from your creatinine along with your age and sex.
If your creatinine came back high or has been climbing, bring it to your clinician and ask what your eGFR is and whether a repeat test is warranted. The eGFR puts your creatinine in the context of your age and sex, and a single high reading often needs a second test weeks later before it means anything lasting.
What is creatinine in a blood test?
Your muscles run on creatine phosphate, a small energy-storage molecule, and a steady fraction of it breaks down each day into creatinine, a waste product with no further use. Your body makes it at a fairly constant rate that roughly tracks your muscle mass, dumps it into the blood, and the kidneys clear most of it, chiefly by filtering it into the urine, with a smaller share secreted by the kidney tubules 1. Because production is steady and clearance depends on the kidneys, the blood level works as a rough gauge of filtering: when the kidneys slow down, creatinine backs up.
The single number cannot see muscle. A 25-year-old weightlifter and an 80-year-old with thin arms can have identical, healthy kidneys and still post very different creatinine levels, because the weightlifter makes more of it. Read raw, the same result flatters the low-muscle person and looks worse in the muscular one. That is the whole reason labs feed creatinine into an equation instead of judging it directly.

What is a normal creatinine level?
A normal serum creatinine is roughly 0.7 to 1.3 mg/dL for adult men and 0.6 to 1.1 mg/dL for adult women, with women's range lower because they carry less muscle on average 5. Older adults often sit toward the lower end despite reduced filtering, again because they carry less muscle, which is one more reason a lone creatinine can look reassuring when it should not. Every lab prints its own reference interval on the report, so compare your value to the range next to it, not to a friend's.

A normal creatinine is a good start, not an all-clear. In a classic study of patients with kidney disease, serum creatinine stayed inside the normal range until glomerular filtration, the kidneys' actual filtering rate, had fallen to roughly half of normal 3. Clinicians call that gap the creatinine-blind range: the stretch where filtering is already dropping but the blood level has not yet crossed the line. It is why one clean creatinine reading rules little out on its own.

The word that matters there is present for more than 3 months. A single low or high result can come from a passing illness, dehydration, or a drug, so one abnormal creatinine is a reason to repeat the test, not a diagnosis 1.
What does a high creatinine level mean?
A high creatinine means creatinine is building up faster than your kidneys clear it. Reduced filtering is the reason clinicians most want to rule in or out, but it is not the only thing that pushes the number up. Several everyday factors raise creatinine with your kidneys doing nothing differently:
- Dehydration, which concentrates the blood and briefly slows filtering, and is why clinicians often read creatinine next to the BUN:creatinine ratio to tell dehydration apart from a true kidney problem
- A large cooked-meat meal in the day or two before the draw, since cooking converts some creatine in meat to creatinine you then absorb
- Intense exercise shortly before the test
- More muscle mass than average, or creatine supplements
- A few drugs that block creatinine's exit route in the kidney tubules, such as trimethoprim (an antibiotic) and cimetidine (a heartburn drug), which raise the blood level without changing true filtering
Because so many things nudge creatinine, a single high value is usually repeated, and it is read next to the eGFR rather than alone. A result that is clearly above your lab's range, or one that keeps climbing across tests, is more likely to reflect real filtering loss and belongs with a clinician.
Why creatinine is fed into eGFR
Because the raw creatinine cannot separate a big body from a struggling kidney, labs plug it into the CKD-EPI equation along with your age and sex to estimate glomerular filtration rate, or eGFR. The 2021 version of that equation removed a race coefficient it used to carry, so the same creatinine now returns one eGFR for everyone 24. The estimate is not perfect: about 1 in 7 to 1 in 10 results land more than 30 percent from a directly measured filtration rate 2. But it beats reading creatinine cold.

eGFR turns your creatinine, age, and sex into one number estimating how many millilitres your kidneys filter per minute, and below 60 for more than three months is the line that marks chronic kidney disease.
When someone's build makes creatinine hard to trust (a bodybuilder, an amputee, a very frail older adult), a second filtering marker called cystatin C, which muscle does not distort, can confirm or refine the eGFR 6.
How to lower creatinine
If a result was high because you were dehydrated, ate a big meat meal before the draw, or trained hard that morning, a properly timed repeat under normal conditions often reads lower on its own, no intervention needed. What does not work is chasing the number directly: creatinine tracks kidney function, so when filtering is genuinely reduced, the answer is finding and treating the cause with a clinician, not hunting for a food or supplement to push the lab value down.
Anything more specific, such as changing a medication that may be raising it, adjusting protein intake, or treating the high blood pressure or diabetes that damage kidneys over time, is a decision for you and your clinician using your full picture, not a number to self-manage.
How it is tested and when to retest
Creatinine is a standard part of a basic metabolic panel and a comprehensive metabolic panel, the routine blood chemistry panels ordered at most checkups, so you usually do not order it separately, and most labs report the eGFR next to it automatically. It does not require fasting, though heavy exercise and a large meat meal shortly before the draw can nudge it up. The test is inexpensive and typically covered as routine bloodwork.
People with diabetes, high blood pressure, heart disease, or a family history of kidney disease are usually checked at least yearly, because those conditions are the common drivers of kidney damage. Any abnormal result is repeated to see whether it persists, since the definition of chronic kidney disease requires the change to last more than 3 months 1.
Creatinine and its eGFR are only half of the kidney picture. As the LifeFrom kidney overview puts it, kidney function is scored on two axes at once, how fast you filter (eGFR) and how much protein leaks into your urine (albuminuria), because damage often shows up in the urine before the filtering number drops. A normal creatinine with protein in the urine still counts as kidney disease.
