When is cystatin C ordered?

Cystatin C is a separate blood test, not part of the standard metabolic panel, so a clinician has to order it deliberately. The clearest trigger comes from the KDIGO kidney guidelines: when a creatinine-based eGFR lands in the 45 to 59 range (a borderline zone called G3a) and there is no other sign of kidney damage, they suggest measuring cystatin C to confirm whether chronic kidney disease is really present. 2 The 2021 US task force that removed race from the eGFR equation went further, recommending wider use of cystatin C to confirm filtering rate in adults at risk for or living with kidney disease. 4

Cystatin C tends to change the answer when creatinine is the unreliable one:

  • Very muscular people, whose creatinine reads falsely high, so their eGFR looks falsely low
  • Frail, older, or very ill people with little muscle, whose creatinine reads falsely low, so their eGFR looks falsely reassuring
  • Amputees and people with muscle-wasting conditions
  • People with cirrhosis (liver scarring), and some athletes on high-protein or creatine-heavy regimens
  • Anyone whose eGFR sits right on a decision line and needs confirming

What does a high cystatin C mean?

A raised cystatin C usually points to filtration that is slower than normal, and because muscle does not distort it, that signal is often more trustworthy than a single creatinine. 1 It is still one reading. Chronic kidney disease is defined by an abnormality that persists beyond three months, so a lone high value needs a repeat, and short-term causes such as dehydration, an infection, or a new medication have to be ruled out first. 2

Cystatin C is a standard blood draw with no fasting required.
