A white blood cell count of, say, 13,000 means completely different things depending on which cell type pushed it up: neutrophils point to bacterial infection, stress, or steroids; lymphocytes to a virus; eosinophils to allergy or parasites; monocytes to chronic inflammation. Reading the total without the differential throws away the actual signal.
Your white blood cell (WBC) count is the number of infection-fighting immune cells in a set volume of blood, and on a lab report it usually lands somewhere near 4,500 to 11,000 cells per microliter (a very small volume of blood) 16. The single total is the least interesting part. What tells the story is the differential: which type of white cell is high or low.
What is the white blood cell count, and how is it measured?
Your white blood cell (WBC) count is the number of white blood cells, the immune cells that fight infection, in a set volume of blood. It is one line on the complete blood count (CBC), the standard panel that also reports red cells and platelets 2. A CBC is three tests in one panel: red cells carry oxygen, white cells fight infection, and platelets clot. The WBC count is not a separate order; it comes with the CBC, one of the most common and least expensive blood tests, and it needs no fasting 2. Most panels report the differential automatically. If yours shows only a total with no breakdown, the differential can usually be run on the same blood sample.
What is a normal white blood cell count?
A normal total white blood cell count in adults runs about 4,500 to 11,000 cells per microliter, a microliter being a volume smaller than a single drop of blood 16. Exact cutoffs vary from lab to lab, so a value that sits just outside the printed range is often not clinically meaningful on its own. What counts is how far outside it sits, whether the differential is normal, and whether the trend over time is stable or moving.

Why the differential matters more than the total count
The differential is the breakdown of the total into the five kinds of white cell: neutrophils, lymphocytes, monocytes, eosinophils, and basophils. That split is the real read. A count pushed up by neutrophils (bacterial infection, inflammation, physical stress, steroids) means something completely different from one pushed up by lymphocytes (many viral infections), eosinophils (allergy, drug reactions, or parasites), or monocytes (chronic infection or inflammation) 15. A report that shows only a total, with no differential, answers a much smaller question than one that shows the breakdown 2. The two lines that most often carry the signal have their own pages. Per cubic millimeter (mm3) is the same as per microliter (µL), so that 11,000 cutoff is the same number a US lab prints. The table below is the map of what a shift in each line tends to suggest.

What does a high white blood cell count mean?
A high white blood cell count is called leukocytosis, defined as a total above about 11,000 cells per microliter in non-pregnant adults 1. Most of the time it is a normal reaction rather than a disease: infection, inflammation, physical or emotional stress, a hard workout, smoking, and pregnancy all raise the count, and steroids do too. Neutrophils are the usual driver, which is why a raised total with a neutrophil-heavy differential most often traces back to a bacterial infection or inflammation 1. The counts that change the picture are the extreme ones. A total above 100,000 cells per microliter is almost always leukemia or a myeloproliferative disorder (a bone-marrow condition that overproduces blood cells), not a simple infection 14.
Common causes of a high count
- Infection, especially bacterial (neutrophils up)
- Viral infection (lymphocytes up)
- Inflammation or a recent injury
- Physical or emotional stress, and strenuous exercise
- Smoking and pregnancy
- Steroids and some other medications
- Allergy, asthma, or a parasite (eosinophils up) 5
- Less often, leukemia or a myeloproliferative disorder
What does a low white blood cell count mean?
A low white blood cell count is called leukopenia. The neutrophil count matters more than the total here, because neutrophils are the front-line defense against bacteria. An absolute neutrophil count (the actual number of neutrophils, or ANC) below 1,500 cells per microliter is neutropenia, and below 500 is severe neutropenia, which carries a real risk of serious infection 3. Common causes of a low count include some viral infections, chemotherapy and certain other medications, autoimmune conditions, and shortages of vitamin B12 or folate. How often should you retest a white blood cell count?
For a mildly abnormal, isolated result in someone who feels well, the trend matters more than one reading, so a repeat CBC in a few weeks is often all that is needed to see whether the count settled or is drifting. There is no routine WBC screening for healthy adults; the count is measured as part of a CBC ordered for symptoms, monitoring, or before a procedure. A result that is far outside the range, that comes with symptoms, or that shows an abnormal differential is the one to act on sooner.
