Most of the B12 in your blood is bound to haptocorrin, a protein that never delivers it to your cells, so the total can read normal while the usable fraction runs low. In the 200 to 300 pg/mL gray zone, methylmalonic acid and homocysteine, which rise only when cells run short, settle what the total cannot.
A vitamin B12 blood test usually reports total serum B12, and most US labs flag anything under about 200 pg/mL (148 pmol/L) as low and anything above roughly 300 pg/mL (221 pmol/L) as normal.1 That single number hides a problem. It counts all the B12 in your blood, including the large share your cells can never use, so a result that looks fine can still miss a deficiency.

Reference ranges vary a little between labs, but the shape holds: under 200 pg/mL reads as low, above 300 to 400 pg/mL reads as adequate, and the band in between is where a single number cannot decide. The gray zone matters because total B12 has limited sensitivity there, so a result of 250 pg/mL does not rule a deficiency in or out on its own.1 Check the reference range printed next to your own result, since the exact cutoffs differ by assay. Why a normal total B12 can still be too low
B12's chemical name is cobalamin, and it travels in blood on two carriers. Most of it rides on haptocorrin, which holds onto B12 without releasing it to cells. A smaller share rides on transcobalamin; that fraction, called holotranscobalamin or active B12, is the part cells can take up.2 Total serum B12 adds both together, so a normal total can reflect a full haptocorrin pool while the usable active fraction is running down.

If your total B12 lands in the gray zone, the next step is a functional test that reflects what is happening inside cells. As the methylmalonic acid (MMA) page puts it, "a normal MMA is one of the strongest single arguments against a true B12 deficiency," because MMA climbs only when cells run short of B12.1 Homocysteine moves earlier but is less specific: the homocysteine page notes it "rises with low folate, low B6, or kidney disease as readily as with low B12."1 And the active B12 (holotranscobalamin) page describes it as "the usable fraction bound to transcobalamin that cells can take up," which can fall before the total moves, so some labs offer it first-line.2
How folate can hide a B12 problem
Folate and B12 work together to build red blood cells, so a folate supplement can correct the megaloblastic anemia (large, immature red cells) of B12 deficiency while the nerve damage keeps advancing underneath.4 That is why a clinician checks the two together and treats the B12 problem, not only the blood count. Folate itself comes in two tests that answer different questions: the serum folate page frames it as "what you ate in the past few days," while the red blood cell folate page calls RBC folate "a marker of tissue stores over the previous few months."4
Pernicious anemia, an autoimmune loss of the stomach's intrinsic factor (the protein that lets you absorb B12), is a leading cause of severe deficiency. Anti-intrinsic-factor antibodies are specific but only about 50% sensitive, so a negative test does not rule it out.4 For the full list of causes, symptoms, and management, the B12 deficiency page notes that "most cases trace to absorption problems rather than diet, so finding the cause matters as much as correcting the number."
What do high B12 levels mean?
A high B12 is usually not a sign of good nutrition. The most common cause is recent supplements or a B12 injection, which can push the number well above the reference range for weeks.5 When a high B12 is not explained by supplements, it can flag liver disease, a myeloproliferative or myelodysplastic disorder (bone-marrow conditions), some solid tumors, or kidney failure, so an unexplained elevation should be checked.5 There is no benefit to a higher number for its own sake, and water-soluble B12 from food or standard supplements is not typically stored to toxic levels the way fat-soluble vitamins can be.
What is cobalamin, and how does it differ from methylcobalamin?
Cobalamin is the chemical name for vitamin B12, a family of related molecules built around a cobalt atom. Your body uses two active coenzyme forms: methylcobalamin, which processes homocysteine, and adenosylcobalamin, which works in the cell's energy machinery.3 Most supplements contain cyanocobalamin, a stable synthetic form the body converts to the active ones, and clinics often inject hydroxocobalamin, which lingers longer in the body.3 For reading a blood test, the form you took does not change what total serum B12 measures; the test counts cobalamin regardless of type.
How the test works, and who should check it
- No fasting needed. B12 is a simple blood draw with no special prep, and it can run from the same tube as other bloodwork.
- Separate order. Total B12 is not part of a basic metabolic or lipid panel; it usually has to be requested specifically, sometimes alongside folate.
- Who should check it: people on a vegan or strict vegetarian diet, adults over about 60, anyone on long-term metformin or long-term acid reducers (proton pump inhibitors or H2 blockers), and people with gut conditions or stomach or intestinal surgery, because all of these reduce B12 absorption.3
There is no fixed screening interval for healthy adults; the groups above are the ones most worth watching. If your B12 is low or borderline and you and your clinician act on it, retesting is usually done after treatment to confirm the number has responded, and the underlying cause guides how often to recheck.
