Vitamin B12 deficiency: symptoms, causes, and why nerve damage can come first

The symptoms, the causes, and why a normal blood count is not the all-clear for low B12.

    Nerves can be damaged before anemia shows up

    B12 deficiency is often pictured as an anemia, but the nerve damage can begin, and become permanent, while the blood count and red-cell size still look normal. Nerve impairment shows up with a normal red-cell size in about 1 in 4 cases (British Committee for Standards in Haematology, 2014). That is why a normal blood count does not clear you, and why tingling, numbness, or unsteadiness are the symptoms you act on quickly.

    The same blind spot, one layer deeper: folic acid can mask it

    Here is the same trap from another angle. Taking folate (folic acid) can normalize the size of your red blood cells and erase the anemia of B12 deficiency, but it does nothing for the nerve damage, which keeps progressing. That is why B12 and folate are checked together, and why the fix targets the B12, not the blood count alone.

Vitamin B12 deficiency means your body is short of B12 (cobalamin), the vitamin your nerves and red blood cells depend on. The nerve damage can start, and become permanent, before your blood count ever changes, so a normal count is not the all-clear. 1 That is why tingling, numbness, and unsteadiness are the symptoms to act on quickly rather than wait out.

What are the symptoms of B12 deficiency?

Early on, low B12 often causes nothing you would notice; subclinical B12 deficiency (low markers without symptoms) affects an estimated 2.5% to 26% of people, depending on the definition used. 3 As it deepens, it shows up in two systems: the blood and the nervous system. 1 The blood side is anemia, too few healthy red blood cells, which brings fatigue, breathlessness on exertion, and pale skin. The nerve side is the one to know, because it can appear on its own.

  • Fatigue and weakness that rest does not fix.
  • Tingling, pins-and-needles, or numbness in the hands and feet (called paresthesia).
  • Balance and gait trouble: feeling unsteady or clumsy, especially in the dark.
  • Brain fog, poor concentration, low mood, or memory slips.
  • A sore, smooth, red or 'beefy' tongue (glossitis) and mouth ulcers.
  • Sometimes anemia on a blood test, but not always.

Why nerve damage can come before anemia

B12 keeps two jobs running: building red blood cells and maintaining the myelin sheath, the insulation around nerves. When cells run short of usable B12, both jobs suffer, but not at the same pace or in the same order. Nerve damage, including peripheral neuropathy (nerve damage in the hands and feet) and subacute combined degeneration of the spinal cord (harm to the spinal cord's nerve tracts), can occur with a normal red-cell size and no anemia at all. 25

The British Committee for Standards in Haematology puts the overlap plainly: nerve impairment shows up with a normal mean red-cell size in about 1 in 4 cases. 2 The classic hospital series behind this figure found that many patients with neurological or psychiatric signs of B12 deficiency had no anemia and normal-sized red cells. 5 So a normal complete blood count does not rule out B12 as the cause of your symptoms.

Illustrative. Methylmalonic acid (MMA) climbs as cells run short of B12, and nerve symptoms can begin, well before hemoglobin drops far enough to flag anemia. The anemia line is sex-specific, about 12 g/dL in women and 13 g/dL in men. The shape is grounded in the finding that B12 nerve damage occurs with a normal red-cell size in about 1 in 4 cases (Devalia et al., 2014).

What causes B12 deficiency?

B12 comes almost entirely from animal foods, and absorbing it takes several steps: stomach acid to free it from food, a stomach protein called intrinsic factor to carry it, and a healthy end of the small intestine (the ileum) to take it in. A problem at any step can cause deficiency. 41

Pernicious anemia is the classic cause worth naming, because it is autoimmune and can run in families. 2 If you have it, first-degree relatives with fatigue or nerve symptoms are worth checking too.

How B12 deficiency is confirmed

The usual first test is total B12 in the blood. It does not need fasting and is a routine, low-cost blood draw. 2 The catch is that total B12 measures all the B12 in your blood, including the portion that cells cannot use, so it can read normal while your cells are starved.

A normal total B12 does not rule out deficiency; MMA and homocysteine show whether your cells actually have enough.

When the number is borderline, or when symptoms are strong but total B12 looks normal, two follow-up markers settle it. The folate trap: why B12 and folate are checked together

Folate (vitamin B9) and B12 work in the same pathway to build red blood cells, so a shortage of either can cause the same oversized-red-cell anemia. The trap is that taking folic acid can correct that anemia and normalize red-cell size even when the real problem is B12, while the B12 nerve damage keeps progressing unseen. 14 That is why a clinician checks folate and B12 together, and treats the underlying B12 shortage, not only the blood count.

How is B12 deficiency treated?

Treatment replaces the missing B12, by mouth or by injection, and the right route depends on the cause and how severe things are. Someone with pernicious anemia or gut surgery may not absorb oral B12 well and is often treated with injections; a diet-related shortage may respond to oral B12. 2 The dose, route, and how often to recheck are a clinician's call, set to your cause and monitored over time, so this is not something to self-dose from a supplement aisle.

Who should get checked, and how often

Testing makes sense if you have the symptoms above, or if you carry a risk factor: years on a vegan or vegetarian diet without supplementing, long-term metformin or acid reducers, past stomach or bowel surgery, celiac or Crohn's disease, or a family history of pernicious anemia or other autoimmune conditions. 42 Retest timing depends on why you were low and how you are treated; your clinician sets the interval, often to confirm the level and symptoms are improving after treatment starts.
