What is a methylmalonic acid test?
Methylmalonic acid (MMA) is a small compound your cells produce when they process leftovers from protein and fat breakdown. When vitamin B12 (cobalamin, the active B12 your cells use) is doing its job, an enzyme clears MMA almost completely. When B12 runs short inside the cell, MMA piles up and spills into the blood, where a lab can measure it from a standard blood sample (some labs use urine). A high MMA is a stand-in for a B12 shortage at the tissue level, which is why it can flag a problem before serum B12 falls far or symptoms appear 3.
Why is MMA tested?
Serum B12 is a noisy first-line number. Plenty of results land in a gray zone, roughly 148 to 258 pmol/L (about 200 to 350 pg/mL), where the value neither confirms nor rules out a deficiency 1. What does a high MMA mean?
A high MMA means the B12-dependent reaction inside your cells has stalled and methylmalonic acid is backing up. Paired with a low or low-normal serum B12, that points to a genuine B12 deficiency 2. A normal MMA in someone with healthy kidneys makes a true B12 deficiency unlikely, even if the serum B12 looked a little low.

Read together with homocysteine (a related amino acid that also rises when B vitamins run low), MMA also names the culprit. When both MMA and homocysteine are high, that points to B12; a high homocysteine with a normal MMA points to folate instead. That pairing is how one blood draw separates a B12 problem from a folate problem.

Homocysteine is the other marker that rises in B12 deficiency, and it is a touch more sensitive (it rarely misses a real deficiency), but it is not specific (a high result does not tell you which vitamin is low). It also climbs with low folate, low B6, and kidney or thyroid problems, so on its own it flags that something is off without saying which vitamin is low 1.
Homocysteine rises with low B12, low folate, or low B6, so by itself it flags a problem without naming the cause.
MMA does not have that ambiguity, because only B12 runs the reaction it depends on. That is why it is the marker used to confirm B12 specifically 5.

What is a normal MMA level?
Most labs put the upper limit of normal serum MMA around 0.27 to 0.40 micromol/L (270 to 400 nmol/L), and the exact cutoff depends on the lab and the assay used 4. Read your result against the reference range printed on your own report rather than a single fixed number.

How MMA is tested and who should check
MMA is usually run from the same blood draw as your B12, with no fasting required, though it is a separate order rather than a default on a routine panel. It earns its place when a serum B12 is borderline, when someone has symptoms that fit a B12 deficiency (tingling, numbness, balance trouble, fatigue, a sore tongue) despite a normal-looking B12, or in higher-risk groups: older adults, people on metformin or long-term acid-reducing drugs, vegans and vegetarians, and anyone with a malabsorption condition (a gut problem that blocks nutrient absorption, such as coeliac or Crohn's disease). If MMA confirms a shortage, the underlying cause is worked up as B12 deficiency.
When a high MMA leads to B12 treatment, MMA typically falls within days to weeks once the deficiency is corrected 2, so a repeat MMA is one way to check that treatment is working. Your clinician sets the timing and decides whether a repeat is needed.
