Vitamin B12: how much to take, which form, and whether you need injections

What B12 does, who runs low, and how to take it, including why an oral supplement works for most people and why a blood test comes before the bottle.

    The route is a smaller question than the cause

    Whether you take B12 as a pill or a shot matters far less than why your level dropped. About 1% of any oral dose is absorbed without your body's normal machinery, so high-dose pills correct most deficiencies, and trials suggest they can cover many pernicious-anemia cases when a clinician directs it. Severe or nerve-involving deficiency is typically started on injections. Find the cause with a test, and the route mostly follows from it.

Vitamin B12 keeps your nerves insulated and your red blood cells forming, and your body stores enough to last months, so a true deficiency usually points to a supply problem (too little in the diet) or an absorption problem (your gut cannot take it in). For most people a daily oral supplement covers both, and injections are the exception, not the default.

What does vitamin B12 do?

Vitamin B12 does two jobs you notice if it runs short. It helps build the myelin sheath, the fatty insulation around nerves, which is why a deficiency shows up as numbness, tingling, and balance trouble. It also helps make red blood cells and copy DNA, so a shortage can cause a specific anemia (megaloblastic anemia, where red cells grow large and immature). Who runs low on B12?

A few groups are far more likely to run low, either because their diet supplies little B12 or because their gut cannot absorb it well:

  • Vegans and strict vegetarians: B12 occurs naturally only in animal foods, so a supplement or fortified food is the reliable source.
  • Adults over 50: the stomach makes less acid with age, and food-bound B12 needs acid to be released, so absorption drops.
  • Long-term metformin users: the diabetes drug lowers B12 over years, with a large analysis of the Diabetes Prevention Program finding measurably lower levels on long-term use. 5
  • Long-term proton pump inhibitor or H2 blocker users: acid-suppressing drugs reduce the stomach acid that frees B12 from food. 1
  • People with pernicious anemia: an autoimmune condition that destroys intrinsic factor, the carrier protein B12 needs for normal absorption.
  • People after stomach or ileal surgery: removing the acid-producing stomach or the ileum (where B12 is absorbed) cuts absorption at the source.

B12 for vegans and vegetarians

Plants do not make B12, so a plant-only diet has no reliable natural source. A daily supplement or B12-fortified foods (many plant milks, nutritional yeast, and cereals are fortified) covers the gap. A common approach is a modest daily dose or a larger dose a few times a week, since the passive 1% absorption means bigger, less frequent doses still get in. Check a level rather than assuming your intake is enough.

Should you test before supplementing?

Yes, especially if you have symptoms or fall in an at-risk group. The first test is a serum B12, a common and inexpensive blood test that is often bundled into a general panel. A result near the low end can be misleading, so clinicians add methylmalonic acid (MMA), a substance that builds up when B12 is functionally low even when the serum number looks borderline-normal. Homocysteine is sometimes checked alongside it.

If you start supplementing to correct a low level, a follow-up test confirms it responded. 02What to take and how

How much B12 should you take, and can you overdo it?

The RDA is 2.4 mcg a day for adults, and most B12 tablets contain hundreds of times that (500 to 1,000 mcg is typical) because absorption is inefficient and the body clears what it does not use. There is no tolerable upper limit for B12. The Food and Nutrition Board did not set one because oral B12 has very low toxicity.

The reason a pill can work even when injections seem necessary comes down to two separate routes into the body: the normal, machinery-dependent one, and a small passive one that needs nothing.

Which form of B12 is best?

Cyanocobalamin is the cheapest, most stable, and most-studied form, and it is what most trials used. Methylcobalamin and hydroxocobalamin are the active or natural forms marketed as better absorbed, but for most people the outcome evidence does not clearly favor them over cyanocobalamin. 1 Sublingual tablets, dissolved under the tongue, have not beaten swallowed tablets in head-to-head trials, so form is a smaller decision than dose or consistency. 1 People with certain kidney conditions are sometimes steered away from cyanocobalamin, which is a conversation for their clinician.

Do you need injections, or will pills work?

For most people, high-dose oral B12 corrects a deficiency as well as an injection does. Injections come in when absorption is severely impaired or a level has to be raised quickly under supervision.

B12 is water-soluble, so there is no strict window. Some people take it in the morning because they find it mildly energizing, but any consistent time works. If you take large doses of vitamin C at the same time, spacing them a few hours apart is a reasonable precaution, though the interaction is minor.
