Because hs-CRP measures the same protein that spikes with any infection or injury, a reading taken during a cold, a flare, or after surgery is meaningless for heart risk. A value over 10 mg/L is set aside and repeated about two weeks later, once you have recovered.
- 03A risk marker, probably not the cause
Genetic studies show that being born with genes that raise CRP does not raise heart-disease risk, so the protein is smoke, not fire. Lowering inflammation itself (the CANTOS trial) does cut events, which is why hs-CRP refines risk but never diagnoses.
hs-CRP (high-sensitivity C-reactive protein) is the exact same liver protein as ordinary CRP, read with a more sensitive lab method so it can measure the low levels that track your long-term risk of heart attack and stroke. Under 1 mg/L points to lower risk, 1 to 3 mg/L to average, and over 3 mg/L to higher risk. It only means something when you are otherwise well, because any cold, injury, or flare pushes the number up and makes the reading useless for heart risk.
What hs-CRP measures
C-reactive protein is a protein your liver releases when inflammation is present anywhere in the body, driven mainly by a signaling molecule called interleukin-6 (IL-6). Standard CRP is built to measure the high values seen in infection or injury. hs-CRP measures the identical protein, but the higher-sensitivity assay reliably reads the low concentrations, roughly 0.5 to 10 mg/L, that reflect the quiet, low-grade inflammation linked to artery disease. 2
CRP is a general fire alarm, not a smoke detector for the heart: it confirms inflammation is present and roughly how big, never where or why.
Think of hs-CRP as the same protein read with a finer ruler, and that resolution is what makes it useful. Only at these low values can a lab separate someone at 0.6 mg/L from someone at 2.8 mg/L, a gap standard CRP reports as the same flat result, and that gap maps onto real differences in cardiovascular (heart and blood-vessel) risk.
What is a normal hs-CRP level?
There is no single normal number; hs-CRP is read as a risk band rather than a pass or fail line. The American Heart Association and CDC group results into three tiers for cardiovascular risk: under 1.0 mg/L is lower risk, 1.0 to 3.0 mg/L is average, and over 3.0 mg/L is higher risk. 1

What does high hs-CRP mean?
A high hs-CRP means low-grade inflammation is adding to your cardiovascular risk, on top of what your cholesterol and blood pressure already say. It does not tell you what is inflamed or why, and it does not diagnose heart disease. The main limitation is timing. Because hs-CRP is the same protein that jumps with any infection or injury, a single high value taken while you are unwell says nothing reliable about your baseline heart risk.
This is why an hs-CRP over 10 mg/L is set aside for heart-risk purposes and repeated about two weeks later. A value that high almost always reflects something acute, an infection or injury, rather than the slow smoulder that drives artery disease. 1
A genuinely raised hs-CRP in a well person is a prompt to look at the basics that lower low-grade inflammation, chiefly not smoking, keeping cholesterol and blood pressure in range, staying active, and losing excess weight. A clinician can help you decide which of these matters most for you.
hs-CRP vs CRP: what is the difference?
hs-CRP and standard CRP measure the same molecule; the difference is the sensitivity of the assay and, because of that, the job each does. Standard CRP is the right test when you need to size active inflammation, such as a suspected infection or a flare of an inflammatory disease. hs-CRP is the right test when someone feels well and the question is how much long-term heart risk low-grade inflammation is adding.

What does hs-CRP measure for your heart?
For your heart, hs-CRP reflects the low-grade, body-wide inflammation that helps turn cholesterol deposits into unstable, artery-clogging plaque (atherosclerosis). In the JUPITER trial, 17,802 adults who had normal LDL (low-density lipoprotein, the 'bad' cholesterol) under 130 mg/dL but a raised hs-CRP at or above 2.0 mg/L were given a statin or a placebo. 3
That result put hs-CRP on the risk map. The 2018 AHA/ACC cholesterol guideline lists an hs-CRP at or above 2.0 mg/L as a 'risk-enhancing factor,' one of the inputs that can favor starting a statin when someone sits at borderline or intermediate risk and the decision is otherwise a toss-up. 4 hs-CRP is also one of the inputs in some cardiovascular risk calculators. It refines the picture on top of LDL and ApoB (apolipoprotein B, a count of the cholesterol particles most likely to clog arteries), it does not replace them, and it is one more reason to read it alongside a full cholesterol work-up.
Marker, or cause?
hs-CRP predicts heart risk, but the CRP protein is probably not what causes the damage. Genetic studies (Mendelian randomization, which uses inherited gene variants as a natural experiment) show that people born with genes that raise their CRP do not have more heart disease. 5 CRP is the smoke, and inflammation is the fire. Proof that the fire itself matters came from CANTOS, which lowered inflammation with canakinumab (an antibody against the inflammatory signal interleukin-1β) without touching cholesterol, and still cut major cardiovascular events by about 15%. 6

How it is tested and when to retest
The single most important rule with hs-CRP is to measure it when you are well.
- 1Test when you are wellWait at least two weeks after any cold, flu, injury, vaccination, or dental or other procedure. Active inflammation of any kind swamps the number and makes it meaningless for heart risk.
- 2No fasting requiredhs-CRP does not need a fasting sample, though it is often drawn with a lipid panel that does. It is usually a separate line on the order and an inexpensive add-on to routine bloodwork.

Who should consider hs-CRP
hs-CRP is most useful for adults at borderline or intermediate heart risk, where the decision to start prevention is genuinely uncertain and one more input could tip it. 4 It adds little for people already known to be at high risk who will be treated regardless, or for people at very low risk with no reason to look. It is not a routine screen for everyone, and it is not a test for chest pain or an emergency.
