LDL-C measures the weight of cholesterol packed inside your LDL particles, but plaque is driven by the number of particles, and every one of them carries an apoB tag you can count directly. The two numbers usually agree; when triglycerides are high or the particles are small and cholesterol-poor they split, so a normal-looking LDL-C can sit on top of a high particle count that is the truer read of your risk.
LDL cholesterol is the cholesterol carried by low-density lipoproteins, the particles that ferry cholesterol through your blood and, when there are too many of them, leave it in artery walls as plaque. It is the one number on a standard lipid panel that treatment guidelines set a target for, because lowering it lowers the rate of heart attacks and strokes in direct proportion to how far it drops.
What to do with your result: an LDL under 100 mg/dL is optimal, 130 to 159 is borderline high, and 190 or above is a red flag worth a prompt call to your clinician. Bring the number to whoever ordered it, ask how it fits your overall heart risk, and if your triglycerides (a fat that travels in your blood) are also high, ask whether an apoB or non-HDL cholesterol check (non-HDL adds up all your artery-clogging cholesterol) would read your risk more accurately. What is LDL cholesterol in a blood test?
Cholesterol does not dissolve in blood, so your body packages it into particles wrapped in protein, called lipoproteins, to move it around. Low-density lipoprotein (LDL) is the main particle that carries cholesterol out to your tissues. 'LDL cholesterol,' or LDL-C, is a measure of how much cholesterol those particles are carrying. When too many LDL particles crowd the bloodstream, they lodge in artery walls and build the fatty plaque that narrows arteries and sets up heart attacks and strokes. That is why LDL is called the 'bad' cholesterol, though the label oversimplifies, as the causality section below explains.

These bands describe the general population. Your personal target depends on your risk. Someone who has already had a heart attack, or who has diabetes, carries far more risk from the same LDL number, so guidelines push their target well below 100 mg/dL 1. A lab may only flag 'high' past a fixed cutoff, but the preventive target for a higher-risk person sits lower than the lab's flag. Is LDL 'good' or bad? What the causality evidence shows
LDL is the one lipid where cause, not just correlation, is settled. Two independent lines of evidence point the same way: people born with gene variants that keep LDL low have less heart disease across their whole lives, and drug trials that lower LDL lower heart attacks and strokes. The effect depends on both how high the LDL is and how long you carry it, which is why lifelong exposure matters, not just today's number 4.

Lower has kept being better. A major trial that used a non-statin drug to lower LDL into the 30s mg/dL still cut the rate of heart attacks and strokes 7.
What does a high LDL mean?
A high LDL means more cholesterol-carrying particles are circulating and able to lodge in artery walls, adding to plaque year after year. The damage is cumulative, which is why a high number matters even when you feel fine. One raised reading is a reason to look at your full risk picture, not a crisis, with a single sharp exception at the top of the scale.
The honest caveat: your LDL number is usually calculated
On a standard panel, the lab does not measure your LDL cholesterol directly. It measures total cholesterol, HDL, and triglycerides, then estimates LDL with a formula (the classic one is the Friedewald equation). The estimate is good enough most of the time, but it drifts. When triglycerides climb past about 400 mg/dL, the calculation breaks down and understates LDL, and at low LDL levels it can be off as well. A newer method (the Martin-Hopkins estimate) is more accurate, and some labs measure LDL directly 5.
The bigger gap is what LDL-C measures in the first place. LDL-C is a weight: the total cholesterol packed inside your LDL particles. What drives plaque is the number of particles, because each particle can lodge in an artery wall regardless of how much cholesterol it happens to carry. Every artery-clogging particle carries exactly one apoB tag, so an apoB blood test counts particles directly. Usually LDL-C and particle count move together. They come apart when particles are small and cholesterol-poor, which is common when triglycerides are high or blood sugar is running high. Then you can have a normal-looking LDL-C sitting on top of a high particle count, and the particle count is the truer read of your risk 6.

How is LDL cholesterol tested?
LDL comes as part of a standard lipid panel, alongside total cholesterol, HDL, and triglycerides, so you do not need a separate order for it. Fasting used to be standard, but most people no longer need to fast for a lipid panel; a non-fasting sample is fine for routine screening 1 (fasting still helps when triglycerides are very high or LDL is being calculated). How often should you get LDL checked?
Low-risk adults are generally screened every 4 to 6 years from their 20s, and more often (yearly or so) with high LDL, heart disease, diabetes, or a family history of early heart attacks 1. If you start a lifestyle change or a medication to lower LDL, it is usually rechecked in about 6 to 12 weeks to see whether it worked 1. If your LDL is 190 mg/dL or higher, first-degree relatives should be checked, because FH runs in families and children in an affected family may be screened earlier than usual.
How to lower LDL cholesterol
Lowering LDL comes down to two levers: what reaches your bloodstream, and, for higher-risk people, medication a clinician decides on. For the first lever, guidelines and dietary evidence point to these steps:
- Replacing saturated fat (fatty and processed meats, butter, full-fat dairy, tropical oils) with unsaturated fats from nuts, olive oil, and fish. Saturated fat raises LDL more than the cholesterol you eat does.
- Adding soluble fiber, such as oats, beans, lentils, barley, apples, and psyllium, which binds cholesterol in the gut and lowers LDL modestly.
- Losing excess weight and staying active, which lowers LDL while improving triglycerides and blood sugar at the same time.
- Not smoking. It does little to LDL itself, but it lowers the overall heart risk that a high LDL feeds into.
For people whose risk is high enough that diet and weight changes are not sufficient on their own, guidelines make statins the mainstay of LDL-lowering medication, with several others (ezetimibe, PCSK9 inhibitors, bempedoic acid) added when needed 1.
