Total cholesterol adds your artery-clogging LDL and your protective HDL into a single figure, so a high total can come from either one. The number on its own cannot tell you which, which is why clinicians read past it to LDL, non-HDL, or apoB. The risk lives in how the total splits.

US guidelines stopped treating a total-cholesterol number as a goal to hit. They estimate your overall 10-year risk of heart disease and act on LDL or non-HDL cholesterol, so a good-looking or bad-looking total is a starting point for the conversation, not the answer.
Total cholesterol is a single number that adds up the cholesterol in every fat-carrying particle in your blood, which makes it the quickest screen on your lipid panel and the least useful one on its own. It lumps your LDL cholesterol, the kind that builds artery plaque, together with your HDL cholesterol, the kind that clears cholesterol away, so a high total can mean you carry a lot of harmful LDL or a lot of protective HDL, and the total by itself cannot tell you which.
What to do with a total cholesterol result: read it alongside the parts underneath it, not on its own. Look at your LDL, your HDL and your triglycerides, or ask your clinician for your non-HDL cholesterol, which is your total minus your HDL and strips the protective part back out. Bring the whole panel to your next visit rather than acting on the headline figure.
What is total cholesterol, and what does it lump together?
Total cholesterol is the amount of cholesterol carried inside all of your lipoproteins added together. Lipoproteins are the particles that move fat through your bloodstream, because cholesterol does not dissolve in water and needs a carrier. Your lab sums three of them: LDL cholesterol (low-density lipoprotein, which drives plaque), HDL cholesterol (high-density lipoprotein, which carries cholesterol back to the liver), and VLDL cholesterol (very-low-density lipoprotein, which ferries triglycerides). As a rough equation, total cholesterol equals your LDL plus your HDL plus about one-fifth of your triglycerides, because VLDL cholesterol is usually estimated as triglycerides divided by 5 rather than measured directly. 2

What is a normal total cholesterol level?
On a standard US lipid panel, total cholesterol is sorted into three bands set by the National Cholesterol Education Program. 1

These bands are a screening scale, not a target to hit. Current US guidelines from the American Heart Association and American College of Cardiology no longer treat a total-cholesterol number as the goal; they estimate your overall risk of heart disease and stroke over the next 10 years, then use LDL or non-HDL cholesterol to guide and track any treatment. 3 A desirable total under 200 is reassuring, but it can still sit on top of a low HDL or high triglycerides, so read the parts even when the total looks fine. There is no separate optimal total to aim for beyond keeping your risk-driving fractions low.
Is high cholesterol always bad?
No. A high total cholesterol is a reason to look closer, not a verdict on its own. Because the total includes protective HDL, some people read above 200, or even above 240, mainly because their HDL is high, which does not carry the same risk as the same total driven by high LDL. The only way to tell the two apart is to read the fractions, and that is where an identical total splits into very different pictures.
Same total of 240, three different stories
Non-HDL cholesterol does this sorting in one subtraction. It is your total minus your HDL, so it drops the protective part and leaves every artery-clogging particle behind: your LDL plus the cholesterol in triglyceride-rich VLDL and its remnants. 4 In the three examples above, the identical total of 240 splits into a non-HDL of 200, 150 and 195, which lines the risk up in a way the total never could.
The 2018 AHA/ACC cholesterol guideline treats non-HDL cholesterol, which combines the cholesterol carried in LDL and VLDL, as more damaging to the artery wall than either particle on its own, and that is why it counts both in a single number. 3
Total cholesterol vs LDL, non-HDL and the ratio
LDL cholesterol is the single largest and most harmful slice of the total, and it is the main treatment target in US guidelines, which is why the question of total cholesterol versus LDL comes up so often. The total includes your LDL but also everything else, so LDL, non-HDL cholesterol, and your total-to-HDL ratio each read risk more sharply than the total alone.

The total-to-HDL ratio is one way clinicians put the split back into a single number: a total of 240 with an HDL of 40 gives a ratio of 6, while the same 240 with an HDL of 90 gives a ratio of about 2.7, and the lower ratio generally signals lower risk. It is a shortcut clinicians use, and it does not replace reading your LDL and non-HDL.
For an even more direct read, some clinicians order apolipoprotein B (apoB), which counts the actual number of artery-clogging particles rather than the cholesterol packed inside them; guidelines list it as an alternative to non-HDL that can sharpen the picture when triglycerides are high. 4

How total cholesterol is tested, and how often
Total cholesterol is not a separate order. It comes as part of a standard lipid panel, a single blood draw that also reports your HDL, LDL and triglycerides, and it is one of the most common and inexpensive blood tests, usually covered by insurance as routine screening. You do not have to fast for total cholesterol or HDL; only the triglycerides, and the LDL and VLDL calculated from them, shift with a recent meal, and expert consensus now supports non-fasting lipid panels for most people, so a non-fasting result is still readable. 5 For screening, US guidance suggests most adults have a lipid panel every 4 to 6 years, and more often when you have risk factors such as a family history of early heart disease, high blood pressure, diabetes, or a previously high result. 6
