On a standard lipid panel, no one measures your VLDL. The lab measures your triglycerides and divides by 5, then prints that as VLDL cholesterol, so the number tracks your triglycerides almost exactly and you move it by moving them. It still matters, because VLDL and its leftover 'remnant' particles are artery-clogging, one reason high triglycerides raise heart risk. You don't chase it on its own, though: non-HDL cholesterol (total cholesterol minus HDL) already folds VLDL into a single artery-clogging number, so read your VLDL as a hint about triglycerides and act on triglycerides and non-HDL with your clinician.
VLDL cholesterol is the cholesterol carried in very-low-density lipoprotein (VLDL), the particle your liver builds to move triglycerides, the main fat in your bloodstream. On almost every standard lipid panel, that VLDL number was never measured in a lab. It was calculated by taking your triglycerides and dividing by 5. So VLDL cholesterol is less a separate test than a restatement of your triglycerides, and it earns its place on the report because VLDL and the leftover particles it becomes are artery-clogging. 1 2 3

What is VLDL cholesterol?
A lipoprotein is a protein-and-fat package that ferries fat through your blood, because fat won't dissolve in water. VLDL is the package your liver loads with triglycerides and sends out to your tissues for energy or storage. The cholesterol riding along in those packages is what your report labels VLDL cholesterol. It is a small share of your total: the liver makes VLDL mainly to move fat, not to deliver cholesterol, so VLDL carries far less cholesterol than LDL does. 2

What is a normal VLDL level?
A usual VLDL cholesterol is about 2 to 30 mg/dL. 2 Because it is triglycerides divided by 5, each VLDL tier lines up with a triglyceride tier. There is no separate 'VLDL goal' in the guidelines; the categories below are really the standard triglyceride categories, converted.

What does a high VLDL mean?
A high VLDL means high triglycerides, because one is calculated from the other. So a raised VLDL is not a distinct problem to solve; it is a pointer telling you your triglycerides are up. High triglycerides are commonly linked to things like excess refined carbohydrate and alcohol, insulin resistance, an underactive thyroid, or genetics, which is why the useful next step is to look at the triglyceride number and the whole panel with a clinician rather than the VLDL line alone.
VLDL vs LDL: what's the difference?
VLDL and LDL are both artery-clogging (that is, they can build the fatty plaque that narrows arteries), but they do different jobs and sit at very different scales. LDL is the main cholesterol-delivery particle and the primary target when lowering heart risk; VLDL is the triglyceride-carrier and a small slice of the total. Remnant cholesterol: why a triglyceride number earns a place on your report
Once VLDL drops its triglycerides off at your tissues, the particle shrinks into a smaller, cholesterol-rich leftover. The cholesterol in those leftovers, together with the cholesterol still in VLDL, is called remnant cholesterol. Genetic studies by Varbo and Nordestgaard found that higher remnant cholesterol is a causal driver of ischemic heart disease (reduced blood flow to the heart muscle), independent of LDL. 3 That is the real reason a triglyceride-derived number is worth watching.
The 2018 AHA/ACC cholesterol guideline treats non-HDL cholesterol, which adds together the cholesterol carried in both LDL and VLDL, as a measure of overall artery-clogging burden. In plain terms, LDL and VLDL cholesterol counted together do more artery damage than either one alone. 6
Non-HDL cholesterol is one subtraction, total cholesterol minus HDL, and it counts every particle that can build plaque, including the cholesterol in VLDL.
Because non-HDL cholesterol already includes VLDL cholesterol in its running total, VLDL is a detail inside that bigger artery-clogging (atherogenic) number, so reading your non-HDL captures it for you.
How do you lower VLDL?
There is no separate lever for VLDL. Because it is your triglycerides divided by 5, VLDL falls when your triglycerides fall. Any medication decision belongs with your clinician, who will weigh your full panel and overall heart risk, not the VLDL line on its own.
How VLDL is tested (and a note on the estimate)
VLDL cholesterol is not a test you order on its own. It comes with a standard lipid panel, calculated from the triglycerides that panel measures, so there is no extra cost and no separate blood draw. Because triglycerides rise for hours after a meal, a fasting sample (usually 9 to 12 hours) gives a steadier VLDL estimate, though many labs now report non-fasting panels; ask your clinician which yours was.
The divide-by-5 estimate, and the Friedewald LDL calculation that relies on it, becomes unreliable once triglycerides climb above about 400 mg/dL. 1 6 A newer approach, the Martin-Hopkins method, estimates LDL and VLDL more accurately than the fixed divide-by-5, especially when triglycerides are high. 5 If your triglycerides are very high, your reported VLDL and LDL may be rough, which is another reason to read the whole panel with a clinician.
