Remnant Cholesterol
Remnant cholesterol is the cholesterol carried in triglyceride-rich "leftover" particles that remain after your body has used up some of the fat in VLDL and related lipoproteins. It's usually calculated as your total cholesterol minus your LDL and HDL.
Remnant cholesterol has gained attention as an independent contributor to atherosclerosis, because these particles can penetrate and lodge in artery walls much like LDL does. Tracking it can add useful information about your cardiovascular risk, especially when your triglycerides are elevated.
A high remnant cholesterol is associated with an increased risk of atherosclerosis and cardiovascular disease, and it commonly accompanies high triglycerides, insulin resistance, excess weight, or metabolic syndrome. It may signal risk that isn't fully captured by your LDL number alone.
A low remnant cholesterol is generally favorable and reflects lower levels of the triglyceride-rich particles that can contribute to artery plaque.
There's no single universally agreed target, but lower is generally considered better, and values are often kept below roughly 30 mg/dL (0.8 mmol/L); because remnant cholesterol is still an emerging marker, interpret it with your clinician alongside your full lipid panel and personal risk.
