Vitamin D helps your gut absorb calcium and keeps your bones and immune system working, yet almost everyone who reaches for a supplement is guessing at whether they need it. A single blood test, 25-hydroxyvitamin D (the storage form your body carries between sun exposure and doses), answers that. If it comes back low, D3 is the form that raises it most reliably; if it is already fine, more pills do not buy you extra health.
What does vitamin D do?
Vitamin D's main job is helping your intestines absorb calcium and phosphate from food, which is what lets you build and maintain bone. Without enough of it, you absorb less calcium no matter how much you eat, and bones soften over time. It also acts as a signal in immune cells and muscle. Your skin makes vitamin D from sunlight, and you get smaller amounts from oily fish, egg yolk, and fortified foods, which is why levels swing with the seasons and with how much time you spend outdoors.
How much vitamin D should I take?
These are reference intakes for the general population, the amount that covers most healthy people, not a treatment dose. A clinician correcting a genuinely low level may use more for a defined stretch, then recheck; that decision belongs with them, not with a label. 13

Reading the blood number: 20 or 30 ng/mL?
Two expert bodies drew the sufficiency line at different places. The Institute of Medicine (now the National Academies) treats a 25-OH vitamin D of 20 ng/mL (50 nmol/L) as adequate for bone health in most people 2, while the Endocrine Society historically drew its line at 30 ng/mL (75 nmol/L) 6 before its 2024 guideline stepped back from a single target 3. That gap is why one lab flags you and another does not at the same result. For most healthy adults, matching the standard intake and respecting the 4,000 IU ceiling matters more than chasing the higher number.

Vitamin D3 vs D2: which should I take?
There are two supplement forms. D3 (cholecalciferol) is the form your skin makes from sunlight and the one in fish and most over-the-counter pills. D2 (ergocalciferol) comes from yeast and mushrooms and is the common high-dose US prescription. Both raise your blood level, but not equally.

When should I take vitamin D?
Take vitamin D with a meal that contains some fat; because it is fat-soluble, it absorbs better alongside food than on an empty stomach. Time of day makes little measurable difference. Daily works, and so does a larger amount taken weekly, because your body stores it between doses.
How and when to take vitamin D
When a lab says you might need it, and who is at higher risk
A low 25-OH result is the signal that supplementing might help. Some people are more likely to land there:
- Little sun exposure: indoor work, northern latitudes, winter, or consistent sunscreen and covering clothing
- Darker skin, because more melanin means less vitamin D made per minute of sun
- Older age, since skin makes less and the kidneys activate less of it
- Higher body weight, because vitamin D distributes into fat and reads lower in the blood
- Malabsorption conditions such as celiac disease, Crohn's disease, cystic fibrosis, or a history of gastric bypass, which block fat absorption

How is vitamin D tested, and how often?
The test is 25-hydroxyvitamin D, a standard blood draw with no fasting required. It is usually a separate order rather than part of a basic metabolic panel, and insurers often cover it only when you have a risk factor or symptom. Paid out of pocket, the price varies widely by lab and region. After starting a steady dose, blood levels climb for weeks before leveling off, so a recheck is timed near 3 months, not right away.
What the evidence supports
The evidence is strongest where the biology is clearest: correcting a genuine deficiency and supporting bone health. For the broader claims, preventing cancer, heart disease, or lifting mood in people who are not deficient, the results have been weak or flat. The largest trial makes the point.

In 2024 the Endocrine Society went further and recommended against routine 25-OH testing in healthy adults, and against vitamin D above the standard reference intake for generally healthy people aged 18 to 74. It reserved extra vitamin D for specific groups such as children, pregnancy, adults 75 and older, and people with prediabetes. 3 In practice, if you are not in a higher-risk group and your level is fine, a bigger dose is unlikely to add anything.
