SHBG, short for sex hormone-binding globulin, is a carrier protein your liver makes that binds testosterone and estrogen and keeps them parked in your bloodstream until a tissue needs them. It decides what share of your sex hormones stays free and usable, which makes it the number that tells you whether a normal-looking total testosterone is genuinely normal or quietly low.

What is SHBG and what does it do?

SHBG is a glycoprotein (a protein with sugar chains attached) that your liver secretes into the blood. It binds the two main sex steroids, testosterone and estradiol (the principal form of estrogen), and it grips testosterone more tightly than estradiol. Because of that grip, circulating hormone splits three ways: a tightly bound SHBG portion tissue cannot easily reach, a loosely bound albumin portion, and a tiny free portion. 15

What does a high SHBG mean?

A high SHBG means more of your testosterone is locked up and less is free. Because SHBG binds testosterone tightly, a rise in SHBG lowers free testosterone more than it lowers free estradiol. The common drivers are older age, an overactive thyroid (hyperthyroidism), liver disease, and estrogen exposure (oral estrogen, pregnancy, oral contraceptives). 1 The result is that total testosterone can read inside the reference range while the free, active fraction is low, which is how a man with real symptoms can be told his testosterone is normal.

What does a low SHBG mean?

A low SHBG means less testosterone is bound and more is free, so total testosterone can read low while free testosterone is preserved. Low SHBG clusters with obesity, insulin resistance (when cells respond poorly to insulin), type 2 diabetes, an underactive thyroid (hypothyroidism), and excess androgens (male-type hormones) or growth hormone. 1 A low SHBG matters on its own, because it tracks with metabolic trouble beyond what it does to testosterone.

SHBG and testosterone: why free testosterone is calculated

Because SHBG decides how much testosterone is free, labs rarely measure free testosterone directly by the gold-standard method (equilibrium dialysis) for routine testing. Instead, free or bioavailable testosterone (bioavailable meaning the free part plus the loosely albumin-bound part your tissues can still use) is usually calculated from three numbers together: total testosterone, SHBG, and albumin. 2 That calculation needs your SHBG as one of its inputs, so the free testosterone number cannot be produced without it.

SHBG is the interpreter sitting between your total testosterone and your free testosterone; when either looks off, SHBG usually explains why. If the free fraction is low and you have symptoms, that is the territory of low testosterone, and it is a conversation for a clinician rather than a self-diagnosis.

Low SHBG and diabetes risk

A low SHBG is more than a testosterone footnote. In a large study of women and men, the people with the lowest SHBG had a much higher risk of developing type 2 diabetes, and the link held even after accounting for the sex hormones SHBG carries and for standard risk factors. 3 Genetic analysis in the same work pointed to SHBG being part of the causal pathway, not merely a bystander.

How SHBG is tested

  • It is a routine blood draw, usually from a vein in the arm. 4
  • Fasting is generally not required for SHBG itself, though it is often drawn in a morning panel alongside total testosterone, which is best measured early in the day. Follow the prep instructions for the whole panel.
  • SHBG can be ordered on its own, but it is most often run as part of a hormone panel or specifically to calculate free testosterone.
  • Reference ranges differ by sex, life stage, and the assay the lab uses, so the only range that applies to you is the one printed on your own report.

Who tends to get it: men with low-testosterone symptoms and a borderline total; women being worked up for excess-androgen symptoms; and anyone whose total sex-hormone result does not fit the clinical picture. There is no fixed screening interval for a healthy person; SHBG is retested when a clinician is tracking something that moves it, such as thyroid treatment, changes in weight and insulin resistance, or starting or stopping estrogen.
