Liver enzymes, explained: how to read a liver function test

The panel most people call a liver function test mostly measures injury, not function. What ALT, AST, and GGT each mean, and how to read the pattern between them.

    ALT, AST, ALP, and GGT tell you liver cells are irritated or leaking, not how well the liver works. Albumin, bilirubin, and clotting time (INR) are the tests that show actual function.

    The size of the rise, not only whether a value is flagged, points to the cause: a mild rise (under about five times the top of normal) is usually fatty liver, while values in the hundreds or thousands narrow to a short, urgent list.

A liver function test is mostly the wrong name for what it does. The common enzymes on it, ALT, AST, and GGT, measure whether your liver cells are irritated or damaged and leaking their contents into your blood, not how well your liver is doing its job. 2 The tests that show real function are albumin, bilirubin, and how fast your blood clots (the INR). What a liver function test measures

The standard panel bundles four enzymes and three function markers. The enzymes (ALT, AST, ALP, GGT) live inside liver cells or the cells lining the bile ducts. When those cells are inflamed, injured, or dying, the enzymes spill into the bloodstream, so a high level signals leak and irritation. 2 It does not tell you the liver has stopped working. For that you look at what the liver makes and clears: albumin (a protein), bilirubin (a yellow pigment from old red blood cells), and clotting factors, measured indirectly by prothrombin time or INR (both are measures of how fast your blood clots).

Grouping the tests by what they answer is the whole trick to reading a panel:

Read it this way and the misnomer fixes itself. If ALT and AST are up but albumin, bilirubin, and INR are normal, the liver is irritated but still doing its work. Enzymes rising while albumin falls or bilirubin climbs is a different, more serious picture, and one to raise with a clinician promptly.

ALT: the liver-specific one

ALT (alanine aminotransferase) is concentrated in the liver and found in little else, so a raised ALT is the closest thing to a direct liver signal on the panel. 1 In a US adult, a mild isolated rise is most often fatty liver. As the ALT marker page puts it, a mildly raised ALT is fatty liver until proven otherwise. How high it climbs matters: the guideline reads a rise as a multiple of the upper limit, from borderline to massive, and the ALT page walks through those buckets. 1

AST: shared with muscle and blood

AST (aspartate aminotransferase) sits in the liver too, but also in heart, skeletal muscle, kidney, brain, and red blood cells, so a high AST is not automatically a liver signal. 1 That is why AST is only informative read next to ALT. The AST marker page makes the point that AST leaks from muscle, heart, and red blood cells as well as the liver, so a high AST with a normal ALT and a raised CK (a muscle enzyme) usually means muscle, not liver. When both are liver-derived, the relationship between them, the AST/ALT ratio, becomes the tell for the cause, covered on the AST/ALT ratio page. 4

GGT: the source-confirming one

GGT (gamma-glutamyl transferase) is sensitive but not specific: plenty of things raise it, so on its own it rarely settles anything. 5 Its real job is confirming where a raised ALP is coming from. ALP (alkaline phosphatase) climbs with both liver and bone problems; a high ALP with a high GGT points to the liver, while a high ALP with a normal GGT points to bone. The GGT marker page sums it up: a raised ALP with a raised GGT is coming from the liver, not from bone. A high GGT alone is not proof of drinking, because obesity, diabetes, and several common medicines push it up too. 5

What are normal liver enzyme levels?

Every lab prints its own reference range, and for ALT many still flag high only above roughly 40 U/L. The 2017 ACG guideline argues that ceiling is too generous, because the healthy reference groups used to set it quietly included people with undiagnosed fatty liver and viral hepatitis, which inflated the printed upper limit. 13

How the test is done, and how often

Liver enzymes usually come as part of a comprehensive metabolic panel (CMP) or a dedicated hepatic function panel, both ordinary, low-cost blood draws often included in routine bloodwork. You do not need to fast for the liver enzymes themselves, though the panel is frequently drawn fasting because it is bundled with glucose and cholesterol. 2

  • Fasting: not required for ALT, AST, GGT, or ALP; fast only if the same draw includes glucose or a lipid (cholesterol) panel.
  • Cost: a CMP is one of the cheaper common labs and is typically covered when ordered for a reason; ask your clinician or lab about self-pay pricing.
  • Rechecking: a mild, isolated rise is often repeated in a few weeks to a few months before any deeper workup, because values fluctuate. 1
  • Who should be screened: people with type 2 diabetes, obesity, or heavy alcohol use have higher odds of fatty liver, and current guidance leans toward checking their fibrosis risk with FIB-4 rather than relying on enzymes alone. 6

What do raised liver enzymes mean?

Most often not much on its own, and rarely an emergency. In a US adult, a mild rise (under about five times the upper limit) is most commonly fatty liver, now called MASLD (metabolic dysfunction-associated steatotic liver disease, previously NAFLD). 1 It is the most common chronic liver condition and, especially in its earlier stages, can often improve or regress with weight loss; the fatty liver page covers the causes, the FIB-4 fibrosis check, and the weight-loss numbers associated with regression. 6 Very high enzymes, into the hundreds or thousands, narrow to a short and more urgent list: acute viral hepatitis, a drug or toxin (including acetaminophen, the active ingredient in Tylenol), or a period of low blood flow to the liver. 12

One habit does most of the work here. Read the enzymes together, then check them against the function markers. Enzymes up with normal albumin, bilirubin, and INR is irritation to explain and monitor; enzymes up while those function markers slip is the pattern that needs prompt attention.
