A high AST on a blood test is not automatically a liver problem. AST (aspartate aminotransferase) is an enzyme that spills into the blood from damaged cells, and those cells sit in your muscle, heart, and red blood cells as well as your liver, so the number only tells a clear story once you read it next to ALT (alanine aminotransferase, the more liver-specific enzyme).
What is AST on a blood test?
AST (aspartate aminotransferase) is an enzyme your cells use to process amino acids, the building blocks of protein. It sits inside cells, so blood levels stay low until cells are injured and spill their contents, and that spill is the signal a blood test picks up. But AST is not unique to the liver: it is also concentrated in skeletal muscle, heart muscle, the kidney, the brain, and red blood cells, so a raised AST can come from any of them. 2
What does a high AST mean?
How high the AST is matters more than the fact that it is up at all. The ACG (American College of Gastroenterology) grades a raised aminotransferase by how many times it sits above the upper limit of normal, and most mild elevations in US adults trace back to fatty liver rather than anything dramatic. 1

Is your high AST even coming from the liver?
Because AST leaks from muscle and red blood cells too, the first question with a raised AST is whether the liver is involved at all. The tell is ALT. When AST is up but ALT is normal, the source is often outside the liver: hard or unaccustomed exercise, a muscle injury, a recent heart attack, or hemolysis (red blood cells breaking down, sometimes just from a difficult blood draw). 2
A simple next check is CK (creatine kinase), an enzyme that rises with muscle damage. A high AST with a normal ALT and a raised CK usually means muscle, not liver. That is why a marathon or a heavy gym session in the day or two before a blood test can flag your AST and mean nothing about your liver. 2
What is the AST/ALT ratio?
AST earns its keep as a ratio with ALT rather than as a lone number. Dividing AST by ALT (the De Ritis ratio, named after the researcher who first described it) sorts the common causes, because different injuries release the two enzymes in different proportions. In most liver disease ALT sits a little above AST, so the ratio lands around or below 1. When the ratio climbs above 2, especially with a raised GGT (gamma-glutamyl transferase), the pattern shifts toward alcohol-related liver injury. The AST/ALT ratio page carries the full read. 3

How to read your AST against your ALT

AST and ALT are usually ordered together and move together, but they answer different questions. ALT is the more liver-specific of the pair, so it confirms that a signal is coming from the liver; AST adds value mainly through the ratio and by flagging non-liver sources. AST also clears from the blood faster, with a half-life near 18 hours against roughly 36 hours for ALT, so AST can fall first as an injury settles. 3

What causes high AST levels?
From the liver
- Fatty liver (MASLD, metabolic dysfunction-associated steatotic liver disease), the most common cause of a mild rise in US adults
- Viral hepatitis, including hepatitis B and C
- Alcohol-related liver injury, classically with an AST/ALT ratio above 2
- Medications and supplements, including some antibiotics, anti-seizure drugs, and high-dose acetaminophen 1
Not from the liver
- Hard or unaccustomed exercise in the day or two before the test
- Muscle injury or disease, including rhabdomyolysis (severe muscle breakdown) and some statin-related muscle effects
- A recent heart attack, which releases AST from heart muscle
- Hemolysis, red blood cells breaking down, sometimes only because the sample was drawn or handled roughly
- Thyroid disease and, occasionally, celiac disease 2

What is a normal AST level?
Most labs put the AST reference range at roughly 10 to 40 U/L, a little lower in women than in men, and the exact cutoff varies with the lab's assay and local population. A reference range is the middle 95% of a healthy group, which means about 1 in 20 healthy people land just outside it with nothing wrong; see how reference ranges work for why a mild flag is often noise.
There is a wrinkle AST shares with ALT: the printed 'normal' ceiling was set decades ago from reference groups that quietly included people with hidden fatty liver, so it runs generous. That critique was documented for ALT 5 and plausibly extends to AST; the lower true-healthy ceiling work was done on ALT (see ALT for those numbers), and the same caution applies to reading AST.
A normal ALT does not clear your liver: the enzyme can read normal while fatty liver, and even advanced scarring, quietly builds.
The same holds for AST. Enzyme height does not track scarring, so a normal AST and ALT can sit over significant fatty liver and even early fibrosis. What does a low AST mean?
A low AST is rarely a problem. Values below the reference range usually mean nothing on their own; they are occasionally seen with vitamin B6 (pyridoxine) deficiency, because AST needs B6 to work. In heavy drinkers, that same B6 shortfall lowers ALT more than AST, which is one reason the AST/ALT ratio rises, not a reason AST itself runs low. A low AST does not need chasing unless your clinician is following a specific concern. 3
How AST is tested and when to retest
AST comes as part of a standard liver panel or a comprehensive metabolic panel (CMP), the routine blood chemistry many clinicians order at a check-up, so you rarely order it on its own. It does not require fasting, though a fasting sample is common because AST is drawn alongside glucose and lipids. As a routine panel test it is inexpensive and usually covered by insurance.
For a mild, symptom-free elevation, the ACG suggests confirming it is real before a big workup: avoid alcohol and hard exercise for a few days, then repeat the test, since a single raised AST can reflect what you did the day before. A persistent elevation is what earns a fuller look. Do not stop a prescribed medication to chase a number without talking to your clinician. This wait-and-repeat approach is for mild, symptom-free results only; jaundice, confusion, or AST in the hundreds needs same-day medical care, not a recheck in a few weeks. 1
