- 01A high GGT is a question, not an answer
On its own a raised GGT does not name a cause, because it climbs with alcohol, fatty liver, and many medications. It becomes useful only when paired with alkaline phosphatase (ALP): a high GGT alongside a high ALP confirms the ALP is coming from the liver or bile ducts rather than from bone. The height of the number does not grade how serious the problem is either, which is why GGT is read as part of a pattern, never alone.
A high GGT (gamma-glutamyl transferase, a liver enzyme) means the liver or bile ducts are under some kind of stress, but by itself it does not say what kind. GGT climbs with alcohol, fatty liver, and dozens of ordinary medications, so the number only becomes useful when it is read next to the rest of your liver panel.
What is GGT in a blood test?
GGT stands for gamma-glutamyl transferase, an enzyme that sits mostly in the cells lining your bile ducts (the small tubes that carry bile out of the liver) and in liver cells themselves. When those cells are irritated or the bile flow is obstructed, GGT leaks into the blood and the level rises. It shows up on liver panels under names like GGT, gamma-GT, or GGTP.
GGT belongs to the cholestatic group of liver tests, the ones that flag problems with bile flow, alongside ALP. What is a normal GGT level?
Typical adult reference ranges run to roughly 40 U/L in women and 55 U/L in men, but GGT is one of the most method-dependent liver enzymes, so the exact cutoff varies by lab and assay 3; here is why reference ranges differ between labs. Always read your result against the range printed on your own report. GGT also drifts up modestly with age and body weight in otherwise healthy people, so a mildly high value is common and rarely an emergency.

GGT has no separate guideline target that sits below the normal range the way ALT does. Lower is broadly better: in a cohort of about 163,944 Austrian adults, higher GGT, even within the normal band, tracked with higher cardiovascular mortality over time 5. That is a population association for your clinician to weigh, not a diagnosis you can read off a single result.

Why a high GGT means little on its own
GGT is the most sensitive of the common liver enzymes, meaning almost anything that stresses the liver or bile ducts nudges it up. That sensitivity is also its weakness: because so many unrelated things raise it, a high GGT in isolation does not point to a cause 12. A normal GGT is reassuring; a high one only starts to mean something read next to the rest of the panel.
The height of the number does not grade how serious the problem is, either. A modestly high GGT can come from a beer habit or an extra 15 pounds, while a much higher one can come from a blocked bile duct. The value is a prompt to look at the pattern, not a severity score.
Clinical guidelines put the same limit in plain terms: the ACG advises against using GGT for the routine evaluation of liver disease and gives it one principal job, confirming that a raised ALP is coming from the liver 1.
What GGT is for: confirming a raised ALP
ALP (alkaline phosphatase) is an enzyme that comes from two main places: the liver and bile ducts, and bone. So when ALP is high on its own, it does not tell you which. GGT settles that. GGT lives in the liver and bile ducts but not in bone, so it acts as the tiebreaker 12.
- High ALP plus high GGT: the ALP is coming from the liver or bile ducts. Common with fatty liver, a partial bile-duct blockage, or certain medications.
- High ALP plus normal GGT: the ALP is coming from bone. Think growth in teenagers, a healing fracture, Paget's disease of bone, or vitamin D problems, not the liver.
- This is why a raised ALP is rarely the end of the story on its own: GGT is what tells your clinician where to look next.

What causes elevated GGT?
The list is long, which is the whole point. These are the drivers you and your clinician will usually sort through, and the clues that separate them.

Beyond these, obesity, type 2 diabetes, and a range of enzyme-inducing drugs all raise GGT even when the liver is otherwise fine 3. That breadth is exactly why an isolated high GGT, with everything else on the panel normal, often needs no more than a repeat test and a look at your medications and habits.
Does a high GGT mean I drink too much?
No. Alcohol is one cause, and a heavy pattern will often push GGT up, but obesity, diabetes, fatty liver, and ordinary prescriptions do the same thing 3. A single high GGT is not proof of drinking, and clinicians know it is too non-specific to use that way.
The alcohol signal shows up across the panel rather than in the GGT number alone. "An AST/ALT ratio above 2 points toward alcohol-related liver injury," as the AST/ALT ratio guide frames it, and a high GGT at the same time reinforces that read 42. GGT and long-term risk
GGT is involved in how the body handles glutathione, one of its main antioxidant systems, which is part of why GGT tracks with oxidative stress and metabolic strain rather than liver injury alone 3. That helps explain the mortality link: across large cohorts, people with higher GGT carry higher long-term cardiovascular risk 5. It is a marker that reflects general metabolic health, not just a liver readout, which is another reason it is read in context rather than alone.
How the GGT test is done
- It is a standard blood draw. GGT is usually bundled into a liver panel or comprehensive metabolic-style panel rather than ordered on its own, though a clinician can add it specifically.
- No fasting is needed for GGT itself. If it is drawn with other tests that do need fasting, follow the instructions for the whole panel.
- As a routine chemistry test it is inexpensive and widely covered when ordered with a medical reason; a repeat to confirm a borderline result is common.
How often to retest, and who should think about it
If a reversible driver is in play, such as alcohol or weight, GGT is worth rechecking after several weeks once that cause is being addressed, since its half-life means it needs a month or more to fall 3. There is no general population screening role for GGT on its own; it is measured when a liver panel is being run or when a raised ALP needs a source. If you have fatty liver (MASLD), your GGT is one of several markers your clinician will follow over time.
