HDL Particle Number
HDL Particle Number (HDL-P) counts how many individual HDL particles are circulating, rather than measuring the cholesterol they carry (standard HDL-C). Like LDL-P, it is typically measured by NMR spectroscopy.
HDL particles help clear excess cholesterol from artery walls, and the count of particles may reflect this protective 'reverse cholesterol transport' capacity better than HDL cholesterol content alone. HDL-P is being studied as a way to refine risk, particularly since HDL cholesterol has proven to be an imperfect and sometimes unreliable predictor on its own.
A higher HDL particle number is generally associated with lower cardiovascular risk, reflecting greater capacity to remove cholesterol from the artery wall. However, HDL biology is complex, and a high number is not a guarantee of protection, it should be read in the context of your whole lipid and risk picture.
A low HDL particle number is associated with reduced protective capacity and higher cardiovascular risk, and often accompanies high triglycerides and insulin resistance.
Higher HDL-P is generally considered more favorable, but because the science is still evolving and very high HDL is not always beneficial, there is no simple target to chase, interpret it with a clinician alongside your other lipids and personal risk. This is an advanced test not on routine panels, usually ordered to clarify borderline or complex risk.
