What ACTH is and what it does

The anterior pituitary makes ACTH by cutting it out of a larger precursor protein, POMC. Once released into the blood, ACTH travels to the adrenal cortex and drives it to produce cortisol, the hormone that manages blood sugar, blood pressure, and the body's stress response. Cortisol then feeds back to the pituitary and dials ACTH down, so the two are meant to move in a loop. 1

Because ACTH is the signal and cortisol is the output, neither number stands on its own. A lab reads them as a pair to locate where a cortisol problem lives. A fourth pattern, a low cortisol with a low or inappropriately normal ACTH (normal when it should be climbing), points upstream to the pituitary or hypothalamus (secondary adrenal insufficiency). Cutoffs per Endocrine Society guidelines. [[1]][[2]]

That fourth pattern completes the grid: a low cortisol with a low or normal ACTH means the adrenal gland is fine but the signal telling it to work never arrived, so the problem is in the pituitary or hypothalamus. In every case the logic is the same, the signal and the output together name the level of the problem. 25

The Endocrine Society's guideline on primary adrenal insufficiency recommends measuring plasma ACTH together with a morning cortisol to establish the diagnosis, which separates a low cortisol caused by the adrenal gland from one caused by the pituitary above it. 2

What is a normal ACTH level?

A morning ACTH usually falls around 7 to 63 pg/mL, but the exact band is assay-specific and set by your own lab, so read your result against the range printed on your report. The tiers below only mean something once a same-morning cortisol is placed beside them. 3

Why draw time and handling change the number

ACTH and cortisol both follow a daily rhythm: highest around 6 to 8 am and lowest near midnight. An ACTH drawn in the afternoon can look low only because of the time of day, which is why the standard is an early-morning draw paired with a cortisol from the same tube. 1

Handling matters as much as timing. ACTH is a fragile peptide that degrades once blood leaves the body and sticks to glass, so it must be collected in a cold EDTA plastic tube (the lab tube with an anticoagulant that keeps the sample from clotting) and spun and frozen quickly. A sample left warm or drawn into the wrong tube reads falsely low, which can make a normal signal look suppressed. 3

How the ACTH test is done

ACTH is a specific, separate order, not part of a routine metabolic or lipid panel, and it is drawn when a cortisol result needs a source. It is a single blood draw, usually in the early morning, sent in a chilled EDTA tube and processed fast. It is almost always ordered alongside cortisol, and often as part of a workup that includes a dexamethasone suppression test (a pill that should switch cortisol off overnight) or a corticotropin stimulation test (a shot that should switch cortisol on) rather than a one-off number. 12

Because ACTH is a send-out or specialty assay at many labs, it can cost more and take longer to result than everyday chemistry, and coverage usually follows a clinical reason for the test. 3

When ACTH is very high: the ectopic clue

Magnitude carries information. A markedly elevated ACTH with a high cortisol shifts suspicion toward an ectopic, non-pituitary source, such as a small cell lung cancer or a carcinoid tumor making ACTH outside the normal loop. A pituitary tumor (Cushing's disease) more often shows a high or only modestly raised ACTH. The size of the number, next to the cortisol, is part of how the source is narrowed down. 1
